Supplemental Materials to be Sent to the Department/School

The following materials should be sent directly to the department/school to which you are applying. For the proper address, check Departmental Information for the program to which you are applying.

If the online Application for Graduate Admission links directly to an online supplemental application for your program, and that online supplemental application collects information duplicative of that requested on the forms listed below (e.g., Statement of Purpose, Supplementary Information), do not fill out those forms again here.

Two official copies of your transcripts may be sent directly from the registrars of the academic institutions you have attended (beyond secondary school), or you may request official copies and send them yourself to the academic department to which you are applying.

If you are a university/college senior, do not wait for senior-year grades before submitting your application and transcript.

UCLA undergraduates please note:   You do not have to submit official copies of UCLA transcripts.

The statement of purpose is very important and is carefully considered in the selection process. Be concise and specific. Your name, proposed major, degree objective, and term for which you are applying must appear on the form.

Three letters of recommendation are required by most departments. Please check Departmental Information for more details. You may download the forms and print them out. To avoid misdirection of the recommendation forms, type, or print clearly, your name and the proposed UCLA major field as they appear on your application.

Applicants are not permitted to inspect letters of recommendation in their UCLA files. If you are admitted to UCLA and enroll, you may then have access to your letters of recommendation unless you previously waived the right of access on the form.

Be sure to use special departmental recommendation forms instead of the forms provided here, if you are instructed to do so by the department.

Complete this form if your transcript is issued under a different name than given in item 1 of the application for admission.

As an additional means of evaluating the qualifications of applicants, a number of departments and schools include the nationally standardized Graduate Record Examination (GRE) among criteria for judgment.

Be sure to list the GRE institution code for UCLA R4837. GRE department codes can be found on the GRE website or by contacting prospective departments. If ETS has not assigned a major code to your proposed area of study, instruct them to send your score to your specific department. Please note that Graduate Admissions/Student and Academic Affairs has no provision for forwarding scores.

Applicants should plan to take the GRE by December to ensure the timely receipt of their test scores for admission consideration.
