Standards and Procedures for Graduate Study
Provides detailed information and sets forth general policies regarding completion of degree requirements, master's
and doctoral committees, examinations and foreign language requirements.
Policies for Thesis & Dissertation Preparation & Filing
Is the official UCLA manuscript preparation guide that contains established criteria for uniformity
in the physical format of theses and dissertations. The regulations included in it supersede any
style manual instructions regarding format. Information on filing dates and procedures, microfilming,
and registration of copyright are also included.
Thesis & Dissertation Meetings
Staff from the University Archives and the UCLA Graduate Division present information on
University regulations governing manuscript preparation and completion of degree requirements.
Students who plan to file a thesis or dissertation during the quarter are encouraged to attend.
Master's Thesis Forms
Nomination of Master's Committee (iPDF-37KB)
Form and regulations governing nomination of a master's thesis committee.
Reconstitution of Master's Committee (iPDF-36KB)
Requires recommendation of the departmental chair or authorized departmental graduate advisor.
Master's Filing Fee Application (iPDF-43KB)
The Master's Filing Fee may be used in lieu of registration if all formal requirements for the degree,
except for filing the thesis or taking the comprehensive examination, are met prior to the first day
of instruction of the term.
Master's Thesis Approval Form (iPDF-37KB)
Form must be presented in duplicate at the time the thesis is filed.
Doctoral Dissertation Forms

Can only be viewed by faculty and staff on our intranet. Used by department staff to notify the Student & Academic Affairs
Office of the results of Oral Qualifying Examination. Students will be advanced to candidacy based on the date the form is
received, provided the information is correct and the examination was conducted in accord with e Graduate Council regulations.
Report on the Final Oral Examination for the Doctoral Degree (iPDF-71KB)
Can only be viewed by faculty and staff on our intranet. Used by department staff to notify the Student & Academic Affairs
Office of the results of the Final Oral Examination for the Doctoral Degree (defense of the dissertation).
Doctoral Filing Fee Application (iPDF-39KB)
The Doctoral Filing Fee may be used in lieu of registration if all formal requirements for the degree, except for taking the final
oral examination (if required) and filing the dissertation, are met prior to the first day of instruction of the term. Doctoral
students may the use Doctoral Filing Fee Application only if they were registered for the previous academic term.

Petitions
Language Petition (iPDF-244KB)
This petition may be used to fulfill or partially fulfill a language requirement.
Please note: if your program requires more than one language, select 'Partially fulfill' for
each requested action. Please consult Program Requirements for UCLA Degrees
for the language requirements your program.
Master's Degree Petition (iPDF-280KB)
This petition may be used to request the following actions: Transfer of Credit, Extension of Time
for Advancement to Candidacy, Waiver of Registration, Plan Change, S/U grade for Graduate Course
Requirement, Change in Mandatory Grading Basis, Unit Deficiency, Application of Units to Concurrent
Degree Programs, Partial Completion of Course Sequence, or Application of 500 Series Courses to
Graduate Requirements. Please consult Standards & Procedures for Graduate Study
and Program Requirements for UCLA Degrees to determine your eligibility for
these actions.
Petition for In Absentia Registration (iPDF-170KB)
This petition may be used to request a reduction of one-half of the campus Registration Fee
for students who need to be registered while studying or doing research outside of California. This
includes recipients of fellowships that require registration to receive financial support or
students conducting fieldwork related to the completion of their graduate degrees.
Survey of Earned Doctorates
The Survey of Earned Doctorates (SED) began in 1958 to collect data continuously on
the number and characteristics of individuals receiving research doctoral degrees from all
accredited U.S. institutions. The results of the survey are used to assess characteristics
and trends in doctorate education and degrees.

For students receiving their doctorates between July 1, 2007 and June 30, 2008. Please submit the
survey to the Graduate Division when you file your dissertation.

UCLA Doctoral Exit Survey
The Doctoral Exit Survey began in 1994 and is used to improve doctoral education at UCLA.
