Must be completed by all U.S. Citizens and Permanent Residents who are recipients of Graduate Division funded awards.

W-9 Form: Request for Taxpayer Identification Number and Certification (iPDF-81KB)
Determines federal tax withholding for US permanent residents.

Where to Return Your Forms

If you are accepting or declining an Offer of Graduate Student Support, return the Graduate Division Copy, Department Copy, and any additional required forms in the envelope provided or in person at 1228 Murphy Hall. Keep the Student Copy for your tax records.

Sign Up for Direct Deposit

All recipients of fellowship awards and traineeships are required to sign up for Bruin Direct to have their stipends deposited directly into personal bank accounts.
